Professional Continuing Education Request for Program Approval Form (Excel form)
This form is to be used by organizations, agencies, or individuals who want to offer a program
to cardiovascular perfusionists that will satisfy part of the continuing education requirements for
recertification.
Continuing Education Recertification Points Request Form (Excel form)
This form is to be submitted for use by the ABCP Continuing Education Committee to document the
Category 1 CEU/SDCE hours available for approved continuing education programs.
